This Form MUST be completed by ANY
and ALL Coaches that may be in the Dug Out or responsible for the
welfare of the team. The team will be billed additional fees if more than 4 are
needed. Only three Coaches will be allowed in Dug Out on game day. Anyone not adhering to Background Check compliance
cannot serve in any coaching and/or supervisory capacity. Volunteer Forms
can be submitted AS SOON AS COACHES ARE DETERMINED. You do not need to
wait until the coaches meeting to submit these forms.
